Private Health Management, Inc. and Private Health Management Medical Group, Inc. (collectively, “Private Health”) respect your privacy and are committed to protecting it. We provide this statement to inform you of our privacy policy and practices for this website. Each client of Private Health also receives a separate notice which describes the way we may use and disclose medical information and their rights and certain obligations we have regarding the use and disclosure of medical information.

Effective June 2008, this privacy policy covers personally identifiable information and anonymous data collection through aggregate reporting. Personally identifiable information is any information that is associated with your name or personal identity (such as your name, address, email address, phone number, etc.). Anonymous (non-personal) data is information about usage and service operation, reported in aggregate, which is not associated with a specific personal identity (such as browser type, IP address, etc.).

From time to time, we may ask you to provide us with personally identifiable information about you through our website. On the Private Health website, you can make information requests or sign up to receive news and information from us.
