Medication Urgency
A very stressed 52-year-old member called us because she was out of an important thyroid medication.
A PHM clinician responded immediately arranging for a prescription of an emergency supply. In parallel, a new endocrinologist and PCP were identified and appointments secured with both, to ensure ongoing medication management.
Comprehensive Care
A healthy 47-year-old member contacted us after an episode of chest pain that required a trip to the ER.
PHM’s clinician discovered she was behind on all healthcare visits and swiftly secured appointments with her PCP, Ob/GYN, orthopaedist and cardiologist. Additionally, due to the ER event, she was also connected a therapist and our clinician remains involved with her care.
Pregnancy Concern
A worried member reached out on behalf of his wife. The couple was nervous after an ultrasound revealed anomalies in their fetus’ femur length and development.
Given the time-sensitive nature of fetal development, PHM organized for a consultation that week with a top geneticist, who offered reassurance. This was followed the next week with another (remote) consultation.
Pediatric Dental Request
The father of a 7-year-old got in touch asking for an emergency dentist evaluation, ideally with a dentist specialising in water laser technology.
PHM managed to identify an expert in water laser technology near the member’s home and secured a same-day appointment. We were pleased to receive super-positive feedback from the whole family.
Orthopedic Injury
A 47-year-old member reached out after an injury during a volleyball game. PHM facilitated an MRI straight away which revealed a torn ACL.
PHM connected him with a knee surgeon, specializing in ACL repair, who performed surgery the next week. Our member is currently doing physical therapy and feeling great.
Neurological Event
A 27-year-old member requested urgent after-hours clinical support with a possible neurologic event/seizure.
A PHM clinician performed intake to assess probability of a serious neurologic episode, providing reassurance and counsel on conservative at home treatment and safety measures. Daily monitoring was then carried out and the member connected with a top epilepsy specialist for further evaluation.
